This summary provides an overview of the benefit programs offered to our full-time employees.
|
ELIGIBILITY
All regular full-time employees are eligible for the benefits outlined in this summary on the first of the
month following 30 days of employment. You are considered to be full-time if you work at least 30 hours
per week and are not a part-time, seasonal, intern, or temporary worker. You may also enroll any eligible
dependents in the medical, dental and vision plans. Eligible dependents include your spouse, domestic
partner, and unmarried children up to the age of 19, or 25 if enrolled as a full-time student.
MEDICAL INSURANCE
- BlueCross BlueShield (BCBS) $750 Deductible PPO
Allows employees to choose between in-network and out-of-network providers to receive care. When
in-network providers are used for routine office visits, benefits are paid at 100% after a $10
co-payment. When in-network hospitals and surgeons are used, benefits are paid at 80% after the
annual deductible has been satisfied ($750 individual/$1,500 family). When out-of-network providers
are used for routine office visits, hospitalization, or surgery, benefits are paid at 60% of the Allowed
Benefit after the annual deductible has been met.
- BlueCross BlueShield (BCBS) 80/60 PPO
Allows employees to choose between in-network and out-of-network providers to receive care. When
in-network providers are used for routine office visits, benefits are paid at 100% after a $10
co-payment. When in-network hospitals and surgeons are used, benefits are immediately paid at 80%.
When out-of-network providers are used for routine office visits, hospitalization, or surgery, benefits
are paid at 60% of the Allowed Benefit after the annual deductible has been met ($300 individual/$600
family).
- Cigna EPP (where available)
Employees must choose a Primary Care Physician (PCP) in the Cigna EPP network and coordinate all
care through this physician. No out-of-network benefits are available in this plan. Specialist care
requires a referral from the PCP. Most services are covered at 100% under this plan after a $10 co-
payment.
PRESCRIPTION DRUG PLAN
- Express Scripts
By enrolling in a medical plan, you will automatically receive prescription drug coverage through
Express Scripts. Generic prescriptions are covered at a $12 co-pay, Brand Name Formulary
prescriptions are covered at an $18 co-pay, and Brand Name Non-Formulary prescriptions are covered
at a $30 co-pay. A mail-order program is also available, which allows employees to purchase a 3-
month supply for the price of two co-pays.
DENTAL INSURANCE
- MetLife PDP
The MetLife Dental Plan offers the flexibility to see any dentist that you choose. If you use a dentist
outside the PPO network, you are responsible for higher out-of-pocket costs. All preventative care
services are covered at 100% in-network. Other basic and major services are covered with an annual
deductible ($50 individual/$150 family) and applicable co-insurance.
- Cigna DMO (where available)
The DMO operates like an HMO in that you will be required to use a Cigna Network General Dentist.
There are no deductibles so coverage begins with the first visit. All preventative care services are
covered at 100%, and all basic and major services have pre-determined co-pays.
VISION INSURANCE
- Vision Service Plan
If you see a participating in-network eye doctor, you pay a set cost for eye exams, lenses, and frames.
You may substitute contact lenses for traditional lenses and frames. You receive a higher level of
benefit by using an in-network doctor. If you see an out-of-network doctor, you will pay for services
out-of-pocket and receive a reimbursement for part of the cost of your exams, glasses, or contacts.
Your out-of-pocket costs will be higher than if you see an in-network doctor.
LIFE AND AD&D INSURANCE
- As a full-time employee of Educate Inc., you will receive one times your base salary in Life Insurance and one times your base salary in Accidental Death & Dismemberment (AD&D) Insurance, at no cost to you, up to a maximum of $250,000. You do have the option to purchase Supplemental Life Insurance on yourself, your spouse, and your dependents and Supplemental AD&D for yourself and your family.
TRAVEL ACCIDENT INSURANCE
- Travel accident insurance protects employees traveling on company business by providing up to
$100,000 of coverage, in addition to your Life/AD&D, for death, dismemberment and permanent
disability.
DISABILITY INSURANCE
- Short Term Disability
Short Term Disability is a company paid benefit that provides income replacement should an employee
become disabled and unable to work. Benefits are paid at 60% of base salary up to $1,000 per week,
for a maximum duration of 12 weeks.
- Long Term Disability
As another company paid benefit, Long Term Disability begins after an employee has been disabled
for 90 days. This benefit replaces 60% of an employee's earnings to a maximum of $6,250 per month.
FLEXIBLE SPENDING ACCOUNTS
- Flexible Spending Accounts (FSAs) allow you to reduce your taxable income by setting aside pre-tax
dollars to pay for eligible expenses for you and your family. You can increase your disposable income
while you cover out-of-pocket expenses. There are two types of FSAs. Health Care FSAs allow you
to receive reimbursement for out-of-pocket health care expenses for you and your family members.
You can elect up to $5,000 each year for your Health Care FSA. You can also elect to contribute up to
$5,000 each year for your Dependent Care FSA to pay for dependent care expenses that allow you and
your spouse to work outside of the home or attend school.
401(k) RETIREMENT SAVINGS PLAN -- Also available to Part-time employees
- By participating in the 401(k) plan, you are saving for your retirement by contributing any percentage
of your pay (not to exceed net take home pay), up to the current annual maximum, on a pre-tax basis.
This means that the amount contributed to the Plan from your pay will not be taxed as income until it is
ultimately distributed to you from the Plan. In addition to your contributions, the company may, at its
discretion, make a matching contribution each plan year.
EMPLOYEE ASSISTANCE PROGRAM (EAP) – Also available to Part-time employees
- LifeBalance
It can be difficult to balance the demands of work with those of your personal life. Stress, work
concerns, and coping with personal situations can throw your life out of balance. LifeBalance is a one-
stop resource that offers consultation, information, and personalized community referrals. It is
available 24 hours a day, 7 days a week for you and your family members. LifeBalance is
confidential, free, and easy to use. This expanded product of our traditional Employee Assistance Plan
will continue to handle all of the traditional EAP issues, plus a wide variety of other services.
TUITION REIMBURSEMENT – Available to Full-time employees only
- Consistent with Educate Inc. Learning System's mission, the company assists its employees who wish to continue their education in order to further their careers within Educate Inc.. The educational assistance program has been designed just for this purpose. The company reimburses up to $2,500 per year of approved educational expenses for courses leading to an undergraduate degree, a graduate degree which is job-required or deemed necessary for an employee's current position, and job-related licensing or certification programs. All college courses must be taken at an accredited
college/university.
